To prepare for your first online session
1) Please download and carefully read the following forms: a) The therapy consent form and b) The telepsychology consent form
Please get in touch if you have any questions or concerns about the information on these forms.
If you do not have any questions about and you consent, please sign the forms and return both of them to me by email before our first session (a clear cell phone photo of each page works fine).
These forms are important because they explain:
This form is confidential and securely stored in your file. It provides me with information about you that helps me assess whether I'm the right person to help and helps us focus and use our time efficiently when we meet.
It's best to complete it in one sitting since it cannot be saved until it's been submitted.
3) Download and create an account for Zoom. Zoom is the platform we will use for our sessions. Shortly before or at the time of our session I will email you a link to allow you to join our online meeting room.

Cancelling, rescheduling, missed appointments
If you need to cancel or reschedule please let me know as soon as possible by calling 780.604.8003 or emailing natsumi@mindbright.ca
I appreciate when cancellations are made with as much notice as possible because it allows me time to offer your session to someone else.
The full session fee is charged for cancellations with less than 24 hours notice, whatever the reason for the cancellation and missed appointment. Most insurance plans will not reimburse for sessions that are not attended.

Payment
Sessions are 50 minutes and the fee is due at the end of each session. The fee is 200 dollars. It is payable by e-transfer or credit card.
